The CPC is the single point of entry to receiving mental health and disabilities services funded by the County. To determine if you are eligible for county funding you must apply to the Community Service office located in the Fayette County Courthouse, 1st floor. What is Case Management ? Case Management is a general term that may be used in a variety of agencies such as DHS, Provider Agencies, and Mental Health Agencies. Fayette County provides a Case Management Unit that assists individuals in attaining resources in the community, and is called a TARGETED TITLE 19 CASE MANAGEMENT SERVICE.
