What is the Central Point of Coordination (CPC) ?
The CPC is the single point of entry to receiving mental health and disabilities services
funded by the County. To determine if you are eligible for county funding you
must apply to the Community Service office located in the Fayette County
Courthouse, 1st floor.
What is Case Management ?
Case Management is a general term that may be used in a variety of
agencies such as DHS, Provider Agencies, and Mental Health Agencies.
Fayette County provides a Case Management Unit that assists individuals
in attaining resources in the community, and is called a TARGETED TITLE
19 CASE MANAGEMENT SERVICE.
Fayette County Targeted Title 19 Case Management Serves:
- Individuals 18 years of age or older, with a diagnosis of mental retardation, mental
illness, or developmental disabilities and have Title 19 (Medicaid) insurance.
- Children only if they are referred through the Department of Human Services and
are on the HCBS MR/Waiver. (You may reach the local DHS at 563-422-5634)
What Does Targeted Title 19 Case Management Do?
- Assists individuals in identifying life desires they wish to achieve.
- Locates Community Resources and Supports to help accomplish the goals
identified.
- Promotes independence and community integration for individuals served.
- Advocates for the rights of individuals as citizens.

office located in the Fayette County Courthouse, 1st floor.
